Make sure all your drivers are up to date. Chipset and graphics drivers are generally the biggest culprits. So check with the manufacturer to see what chipset or "INF" drivers are available.
Okay so i had this problem with one of my monitors and i was stupid when i installed it and plugged it into the first one i saw. Make sure your monitor is plugged into the graphics card not something else like me. Went for 16 - 30 fps to 166 - 200
I'm playing on a 4year old gtx950m laptop ... And i get 60ish fps in open areas and 100ish in dungeons .. ( I've overclocked my gpu though .. and play on lowest and 75% render )
