it is a Crucible reward... you can get it by doing crucible games

I can rule out Omolon that's for sure! it doesn't have the gas cylinder.

It's not Hakke. It's that forge that also makes Nameless Midnight, and it has no designation. Hell, there's some other handcannon with the same model but no Crucible insignia on it, and it's unsigned as well.
Basically, there are two unnamed forges, one that makes most Vanguard weapons (with the exception of Eternal Blazon that I know of, it's an Omolon rifle with Vanguard paintjob), and one that makes guns for the Farm (if it looks like a hodgepodge of AK parts or an Old West revolver, it's the one).
